People with catastrophic injuries require a skilled clinician to ensure that their specific needs are met. They also need someone to help navigate through the health system.

Our healthcare team facilitates each person’s return to function. Experience enables our case managers to anticipate needs well in advance, so that our clients receive the right services and nothing gets missed.

Hospital Discharge Coordination

When a patient is discharged from hospital, their safety and comfort is our focus. Anticipating patient needs in advance helps to safely restore function.

Our goal is to enhance a client’s outcomes after discharge, provide a seamless continuity of care throughout the stages of recovery and lessen the burden on families. We meet with the patient, the family and the treatment team prior to discharge. We make sure that they have the necessary equipment in place and the in-home support that they need to facilitate the transition home.

Attendant Care Needs Assessments

Self care after an injury is important for regaining some measure of independence and privacy.

Our goal is to help clients return to performing as many of their pre-injury personal care tasks as soon possible. We thoroughly assess a client’s personal care needs and determine the appropriate level of support and care required and, when needed, we help to arrange in-home care.

In-Home Treatment

People with acute orthopaedic injuries may need in-home therapy until they can get out to a clinic. Our physiotherapists and chiropractors treat people in their homes.

People with traumatic brain injuries, spinal cord injuries, chronic pain conditions and those with psychosocial barriers often require treatment that helps them transition from home to the community. Our teams of case managers, occupational therapists, physiotherapists and speech language pathologists provide coordinated, integrated care that enables gradual community integration.

For people with chronic pain conditions, we offer the Progressive Goal Attainment Program (PGAP™), which “is the first disability prevention program specifically designed to target psychosocial risk factors for disability. Psychosocial factors were chosen as targets of the intervention on the basis of emerging research supporting their relevance to return-to-work outcomes and their amenability to change through intervention. Life Care Plans / Future Cost of Care Analyses

Complex injuries often lead to a need for long-term or permanent care.

Our life care planners determine the long-term needs of our clients and develop a detailed plan to show what services will likely be needed at the various stages of the injured person’s life. Life Care Plans also provide costs for the various services, as they are required. An actuary or economist then converts the lifetime costs into an equivalent present day cost that an individual can use for planning financial resources, and one that lawyers and insurers can use to negotiate settlements.

In-Home Assessments and Treatment

Sometimes a client can’t leave home for their care because serious injuries restrict a person to bed or home.

Our therapists travel to clients in their homes to determine if they are safe and if any modifications or services are needed to enhance their comfort and safety. We also find out if the person needs treatment delivered in the home or if transportation would enable them to travel to get the care they need. If in-home treatment is required, we put the service in place. We also coordinate care with the whole treatment team, including the family doctor.

Return to Work

Successful return to work depends upon a match between the demands of the job and the worker’s skills. When the two don’t match, therapy may be needed to treat impairments, workplace modifications may be needed to address risk factors and accommodations may be needed to enable optimal function despite functional limitations. And sometimes the worker needs to consider alternate vocational options and maybe even retraining.

Job Site Analyses

Using ergonomic principles, we analyze the work site to determine risk factors for injury, such as excessive lifting, carrying or awkward, sustained postures. We recommend workplace modifications to optimize function and to reduce the risk of injury.

Physical Demands Analyses

Each job has a unique set of physical and cognitive demands. Based on worker and supervisor interviews, observation of another worker in the position and a review of the job description, our therapist determines the specific demands of the job. To obtain objective measures of the job demands, the demands are matched to the National Occupational Classification (NOC) and/or the Dictionary of Occupational Titles (DOT).

Functional Abilities Evaluation (FAE)

The assessment of an individual’s physical abilities is the next step in guiding the need for work conditioning and the timing of return to work. Computerized assessment tools provide accurate determinations of the worker’s functional abilities and his/her ability to safely meet the demands of the job. The results guide rehabilitation and the work conditioning / work hardening program for optimal results.

Workplace Accommodation

When there is a mismatch between the job demands and the worker’s abilities, a number of steps can be taken. The worker can receive therapy to optimize his/her abilities. The job site and/or the job demands can be modified. With skill and knowledge relating to ergonomics, physical and cognitive impairments, as well as workplace legislation, our therapists facilitate worker success through accommodation.

Vocational Assessments/Transferable Skills Analyses/
Market Surveys

Sometimes, despite focused therapy and job accommodations, a person cannot return to the work they once did. That’s when a vocational assessment can identify transferable skills that can be applied to other jobs.

A labour market survey then identifies the availability of the alternative jobs in the worker’s community and the compensation for those jobs.

Thorough and clear reports promote efficient case resolution. We provide objective and reliable opinions regarding the client’s physical, psychological and functional status:
