Physical Impairment

Post-Hospital

Case management to

Occupational therapy
- Home safety assessment to ensure you are secure in your home environment.
- Arrange for the purchase or rental of assistive devices or therapy equipment and facilitate getting it delivered and installed.

- Implement a plan to resume your usual daily activities gradually and safely.
Physiotherapy
- Go to your home to help you build your range of motion and strength.
- Practice navigating stairs inside and outside your home.

Brain Training
We take a personalized approach to brain training. Using the information from our assessment, we develop a therapy program that is fun, interesting and that helps our client address cognitive impairments.
Brain training takes practice which can be supplemented by using:
- Rehabilitation support workers
- PlumTek
- Family members
Spinal Cord Injuries
We provide case management services for individuals who have sustained a spinal cord injury. Our goal is to ensure your safety, quality of care, and cost management.
Our case manager will:
- Work with you to determine care options and to help you plan and put them in place.
- Facilitate technologies:
- Coordinate your care with members of your Circle of Care: medical, therapies, nursing, personal support, family
Virtual Care
- Face-to-face sessions without travel costs.

- To teach skills
